Does my loved one have suicidal tendencies?
For the most accurate results, please answer each of the following questions as truthfully as possible:
1. Have you noticed any sudden significant changes in how your loved one appears to feel (for example, much worse or much better)?
2. Have they recently had a loss? For example, the breakup of a romantic relationship, moving, loss (especially if by suicide) of a friend, loss of freedom, or loss of other privileges?
3. Do they suffer from depression, bi-polar, schizophrenia, substance abuse, eating disorders, and/or severe anxiety?
4. Have they recently been recently diagnosed with physical or terminal illness?
5. Are they currently exhibiting any self-harming behaviors?
6. Have they recently planned to or created a will or are they suddenly visiting friends or family members (one last time)?
7. Are they giving away their personal belongings to friends and family members?
8. Have they recently purchased a gun, hose, rope or medications?
9. Are they currently experiencing a sudden and significant decline or improvement in mood, or writing a suicide note?
10. Have they canceled regularly scheduled meeting with their therapist or psychiatrist for no apparent reason?
11. Have they communicated their plan to anyone? (It is more likely to be someone with whom they are personally close, like a friend or family member.)
12. Are they isolating from their loved ones and normal daily activities?
13. Are they currently experiencing moderate alcohol abuse, insomnia, severe agitation, loss of interest in activities they used to enjoy (anhedonia), hopelessness, and persistent thoughts about the possibility of something bad happening?
14. Are they making suicidal implications such as: the world would be better off without me, my family would be better off without me, etc?
15. Are they referring to their life in the past tense?
